DT-08. Experience in arranging the Irkutsk segment of the Federal Register of Bone Marrow Donors

Summary

Since March 2016, the Hematology Department of the Irkutsk Regional Clinical Hospital has been conducting autotransplantation of hematopoietic stem cells (HSC) for multiple myeloma for the first time in Eastern Siberia, and since 2021, allogeneic transplantation of HSC for acute leukemia.

Results

Currently, more than 100 autologous and 7 allogeneic HSCTs have been performed by the hematologists of Irkutsk Regional Clinical Hospital work closely with the Irkutsk Regional Blood Transfusion Station. The work of hematologists was oriented for bone marrow transplantation practice, thus being the motivation for arranging large-scale recruitment of bone marrow donors among the blood donors of Irkutsk Region. Irkutsk transfusiologists have been cooperating with the Kirov Research Institute of Hematology and Blood Transfusion since 2016 within a research program of studying the genotypes of Russian population. During the working period, we discovered 27 novel HLA gene alleles that have not been previously registered by the World Health Organization committee. In 2019, cooperation with Kirov colleagues was developed, and a decision was made that every donor, following a repeated donation, can join the bone marrow donors Registry. Being connected to the Federal Register of Bone Marrow Donors (Federal Register), the staff of Irkutsk Regional Blood Transfusion Station took a set of measures to promote bone marrow donation among blood donors at the enterprises, offices and educational institutions, as well as in the blood service units of Irkutsk region; a methodology for attracting donors to the Federal Register was developed. The entry of donors into the Federal Register is organized on a permanent basis at all departments of the blood service of Irkutsk region. The Irkutsk segment of the Federal Register of Bone Marrow Donors today includes >15.500 potential donors. Since 2017, medical institutions from Russian Federation, Republic of Kazakhstan, and United Kingdom have sent more than 100 requests for examination of Irkutsk donors, and 26 donors of the Irkutsk segment of the Federal Register have donated HSCs and saved patients in clinics in Moscow, St. Petersburg, Kirov, Yekaterinburg and Astana, for whom the cells were transported after harvesting.

Conclusion

The Federal Register includes donor recruitment centers, typing laboratories, HSC procurement centers and transplantation centers. Combining them in the Federal Register helps to increase the efficiency of interaction between all participants in this process and provides access to the necessary information. Close interaction between hematological clinics and transfusion services in the Irkutsk region makes it possible to imply high-tech methods of treatment, including bone marrow transplantation, for the residents of the Irkutsk Region.
